Then, for any particular title, the company making it might have issued up to a dozen different revisions of the hardware and the rom. So, at least early on, I think (I read this years ago, forgive me if the details aren't perfect) some roms would be dumped only to find out that they weren't dumped correctly. Or the wrong revision was dumped.

And the last few games before this industry died, like Killer Instinct, they actually had a hard drive in them that contained fuck-if-I-know. This was in addition to roms that were necessary to run that game. Tldr It's all very complicated, not entirely the fault of the programmers, and good luck for it ever improving.
